It shouldn’t — proper subcutaneous injection is a straightforward, learnable skill, and the actual risk comes almost entirely from skipping basic sterile practices rather than from the injection itself. This guide covers everything from handwashing to sharps disposal, written the way we’d want it explained to someone doing this for the first time.

Why Sterile Technique Matters

Every injection creates a temporary breach in your skin’s natural barrier against infection. Sterile technique exists to minimize what crosses that breach — bacteria from your skin, from the air, from a contaminated needle, or from improperly stored solution. Done correctly, the risk of infection from a single subcutaneous injection is very low. Done carelessly — reusing needles, skipping alcohol swabs, injecting into compromised skin — that risk climbs quickly and unnecessarily.

None of this requires clinical training. It requires consistency and attention to a handful of steps, every single time, even when it feels repetitive.

Step-by-Step: Subcutaneous Injection

Subcutaneous (SubQ) injection — into the fat layer just beneath the skin — is the standard route for the vast majority of compounds discussed on this site.

Step 1: Wash your hands. Soap and water, minimum 20 seconds, covering all surfaces including between fingers and under nails. This is the single most important step in the entire process and the one most commonly rushed.

Step 2: Prepare your space. Clean, flat surface. Lay out your syringe, alcohol swabs, vial, and sharps container before you start, so you’re not searching for something mid-injection with a loaded syringe in hand.

Step 3: Inspect your vial. Reconstituted peptide solution should be clear and colourless, or very slightly tinted depending on the compound. Cloudiness, visible particles, or an off colour are reasons to stop and not use that vial.

Step 4: Wipe the vial top with an alcohol swab. Let it air dry for a few seconds before puncturing — wet alcohol entering the vial isn’t ideal.

Step 5: Draw your dose. Insert the needle into the vial, invert, and draw back the plunger to the unit mark matching your calculated dose. Check for air bubbles — if you see any, tap the syringe gently to bring them to the top, then push the plunger slightly to expel them before withdrawing the needle.

Step 6: Choose and prepare your injection site. Common subcutaneous sites include the abdomen (at least 2 inches from the navel), the front or outer thigh, and the back of the upper arm. Wipe the chosen site with a fresh alcohol swab in a circular motion, working outward from the centre, and let it air dry completely — injecting through wet alcohol can cause stinging.

Use a 45-degree angle if you have relatively little subcutaneous fat at the site, and a 90-degree (straight in) angle if you have an average amount or more. This distinction exists for a real reason: at 90 degrees with very little fat to work with, there’s a real risk of the needle reaching muscle tissue rather than staying in the subcutaneous layer — the shallower 45-degree angle keeps it where it belongs. Step 9: Inject slowly. Release the pinch and push the plunger down steadily over several seconds rather than all at once — this reduces pressure-related discomfort and helps the solution disperse properly into the tissue.

A simple rotation system: divide your primary sites — left abdomen, right abdomen, left thigh, right thigh, left arm, right arm — and work through them in sequence rather than picking whichever feels convenient that day. Some people keep a simple log or rotation chart taped somewhere private as a reminder, particularly when injecting daily or near-daily.

Give a site time to recover. As a general guideline, avoid returning to the exact same point for at least a few days, and watch for any lingering firmness, redness, or discomfort before using that specific spot again.

Needle Reuse: Don’t

This deserves its own section because it’s the single most common corner people are tempted to cut, particularly when running daily protocols and trying to conserve supplies.

Use one needle, one time, then dispose of it. Never put used needles or syringes in household garbage, recycling, or down the drain. Sharps cannot be recycled, and loose needles in household trash create a real injury risk for waste handlers and anyone else who might encounter them.

Get a proper sharps container. A rigid, puncture-resistant container designed for this purpose — not a repurposed water bottle or jar, which doesn’t reliably prevent a needle from poking through. Sharps containers are inexpensive and widely available.

In Alberta and across most of Canada, community pharmacies are your main disposal channel. Most pharmacies will provide a sharps container at no cost and accept full containers for safe disposal, also at no cost — this is a standard, well-established program, not something unusual to ask about. Which angle should I actually use if I’m not sure how much subcutaneous fat I have? When in doubt, 45 degrees is the more conservative choice, since it reduces the chance of reaching muscle. If you have a clearly noticeable pinch of fat at your chosen site, 90 degrees is standard and generally more comfortable.

Is it normal to feel a small sting during injection? Yes, briefly. Persistent or worsening pain during injection, rather than a brief pinch, is worth stopping for and reassessing your technique or site.

Can I inject through clothing in a pinch? No — always inject into clean, exposed skin that’s been properly swabbed. This isn’t a step to skip for convenience.
